National Register Nomination Forms
Submit this one-page form to SHPO for a quick evaluation of your property's potential eligibility.

The following National Register forms are pre-formatted for use. To use the formatted National Register form, type in the boxes after each entry prompt. Each box is pre-set for Arial font at either 10 or 12 point size. Each continuation section, 7, 8, 9, and 10 of the nomination form has its own document that will automatically add the header to each page with the correct section and page number. The photo continuation sheet and document sheets also have this feature, but have additional pre-formatted blanks in the text. For each pre-formatted form, preparers will need to enter the property name and location information in the appropriate boxes.
